Phosphate Deficit Calculator

Estimate phosphate replacement needs. Approximate — individualize and monitor electrolytes.

Common Formulations

Formulation Phosphorus Potassium (K⁺) Sodium (Na⁺)
K-Phos Neutral (tab) 8 mmol (250 mg) 1.1 mEq K⁺ (45 mg) 13 mEq Na⁺ (298 mg)
Phos-NaK / Neutra-Phos (pkt) 8 mmol (250 mg) ~7 mEq K⁺ (280 mg) ~7 mEq Na⁺ (160 mg)
Potassium phosphate IV 3 mmol/mL (93 mg/mL) 4.4 mEq/mL K⁺ (170 mg/mL) —
Sodium phosphate IV 3 mmol/mL (93 mg/mL) — 4 mEq/mL Na⁺ (92 mg/mL)

Conversion: 1 mmol phosphorus ≈ 31 mg. Serum mg/dL × 0.323 ≈ mmol/L.
IV phosphate must be diluted and infused slowly. Monitor calcium, potassium, and phosphate. Oral preferred when GI function is intact and deficit is not severe. Always individualize.
